All businesses must complete the Wholesale Account Application (US or International) as well as the Terms and Conditions form. You must also provide a copy of your Business/Resell Tax License. License must be issued to the person applying for the wholesale account. Utah-based businesses must also complete the Utah Exemption Certificate TC-721.

Receipt of this information by SWH does not reflect automatic approval for a wholesale account; all information will be reviewed. We reserve the right to cancel an account once established if terms and conditions, both current and future, are not met.

• Internet/Mail Order Business: Initial Order $300, Re-Order $50
